· Works in close collaboration with physicians and other appropriate health care personnel in patient evaluation and treatment to further their understanding of significant social and emotional factors underlying patient’s health problem and assessment of patients’ needs. Consult with professional staff as a member of a multi-disciplinary health care team and participate in case assessment and the development of treatment approach.
· Provides direct services to patients and patient’s relatives through the evaluation of social and psychological data. Helps patient and family through individual or group conferences to understand, accept, and follow medical recommendations and restore patient to optimum social and health adjustment.
· Coordinate external social services and other appropriate assistance for assigned patient caseloads. Provides general direction and supervision to workers engaged in health center home service program activities. Provides service planned to restore patient to optimum social and health adjustment within patient’s capacity. Utilizes community resources to assist patient to resume life in community or to learn to live within limits of disability.
· Interview patients and patients’ relatives to develop and assess patient and family social history. Prepares patient histories, service plans, and reports.
· Identify appropriate community resources such as family and community agencies to provide external patient services. Participates in planning for improving health services by interpreting social factors pertinent to development of program.
· Prepare and maintain appropriate records and reports such as social histories, progress notes, discharge summaries, and the like: review these with relevant medical staff.
· Attends and participates in health center and other ACCESS meetings as required.
· Performs other duties as assigned.
