Position Summary: 
Directs and oversees the provision of and provides quality care to those patients assigned to him/her consistent with guidelines contained within the quality management, risk management, infection control, customer service and safety programs for Access Community Health Network.
Duties and Responsibilities: 

· Meets intensity of service/severity of illness admission criteria.
· Meets continued stay/intensity of service/severity of illness admission criteria.
· Adheres to the hospital guidelines on short stay admissions.
· Adheres to the hospital guidelines on step-down unit.
· Assures the daily health center patient load is seen in a timely and clinically appropriate manner.
· Assures all clinical activity occurring within the health center is accomplished according to acceptable guidelines for reimbursement.
· Assures all daily medical records within the health center are completed prior to the physician leaving the health center.
· Adheres to approved Access Community Health Network treatment protocols.
· Conducts patient education activities, including pre and post instruction exams and procedures, instructions related to illness and treatment such as diet and reinforcement of physician’s follow-up instructions. Utilizes printed health education materials as appropriate and documents all instructions given. Gives consideration to an individual’s varying level of understanding and adjusts level of presentation to ensure the patient is knowledgeable of medical condition and plan of care.
· Assures core team staffs properly carry out effective treatment and education. Assures core team staff performs clinical tasks in accordance with accepted policy and procedure.
· Participates in the orientation and training of staff as needed. Ensures core team staffs meet internal certification requirements for patient education and the administration of medication.
· Provides triage services for emergency walk-in patients, identifying those patients who can wait to receive care and those patients who require immediate medical attention. Schedules service for the patient within a time frame which meets the patient’s medical needs.
· Effectively demonstrates required competencies in the provision of educational, case management, triage and medication administration services.

Requirements: 

- Advanced Study: Broad or in-depth knowledge which would be
acquired and demonstrated through the attainment of a Doctor Osteopathy (D.O.) or a Doctor of Medicine (M.D.)
Degree.
- Completed Residency in Primary Care specialty is required and Board Certification in their primary
care specialty is preferred.
- Two (2) to three (3) years work experience in the
practice of primary care medicine in a freestanding office setting preferred.
