•Performs the following per ACCESS Community Health Network protocols: Histories and physicals on new and return patients; orders appropriate, cost-effective diagnostic and screening studies per ACCESS Community Health Network; makes presumptive diagnosis and implements therapeutic plan; refers to specialty physicians and primary care physicians as indicted; refers to community resources as indicated; provides patient education in disease prevention and wellness promotion; and assures clients medical record meets documentation standards.
•Works with core health team and case management staff to provide for coordinated delivery of case management services.
•Precepts health science students and participates in faculty and university affiliated activities as indicated.
•Initiates and/or participates in ongoing research at ACCESS Community Health Network.
- Current licensure in the State of Illinois as a Nurse Practitioner
- Basic rescuer CPR certification
- Bilingual (English/Spanish) may be required for some sites
