• Works as primary care team member with physicians and other appropriate health providers delivering brief, consultation-based services to patients using an integrated care model in a Federally Qualified Health Center (FQHC).
• Conduct functional assessments to identify barriers in achieving optimal physical health in underserved populations; assist in planning and implementing brief, solution focused interventions and assessing outcomes.
• Consult with professional staff as a member of a multi-disciplinary health care team and participate in case assessment and the development of treatment approach.
• Provides direct services to patients and patient’s relatives through the evaluation of social and psychological data. Helps patient and family through individual or group conferences to understand, accept, and follow medical recommendations and restore patient to optimum mental and physical health.
• Coordinate referral and linkage to external social services for assigned patients when appropriate.
• Identify community resources and establish and foster mutually beneficial relationships with aforementioned resources in order to establish comprehensive patient care.
• Facilitate educational treatment groups focusing on wellness, health promotion and health maintenance.
• Prepare and maintain appropriate records and reports such as assessments, progress notes, etc. and review these with relevant medical staff.
• Attends and participates in health center and other ACCESS meetings as required.
• Performs other duties as assigned.
Master’s Degree from an accredited school of SocialWork or Master’s Degree from an accredited Counseling/Psychology program is required. At least five years of post Master’s degree experience in a health care setting is preferred.
Licensure by the State of Illinois as a Social Worker or Licensed Professional Counselor is required. • Mission: Demonstrates commitment to the philosophy and mission of ACCESS. Applies values and beliefs of the institution to the daily work.

• Demonstrates the ability to interact with team in a medical setting; basic knowledge of medical terms and treatment
• Ability to document clearly, concisely and expeditiously in patient records, formal reports and internal data.
• Knowledge of Joint Commission standards and the ability to apply standards to job responsibilities.
• Able to provide dependable patient services for a at-risk patients to completion; dependability/flexibility to provide back-up coverage as needed.
• Demonstrates knowledge of major activities of the role for the Behavioral Health Consultant and indicates priorities.
• Demonstrates knowledge of services and programs affiliated with ACCESS for patient referrals.
• Demonstrates knowledge of the types of transportation and criteria for use for patients.
• Demonstrates knowledge of the role of utilization review and managed care.
• Documents pertinent Behavioral Health assessments, interventions and outcomes in medical chart.
• Demonstrates knowledge of different types of data to be reported and their significance.
• Participates in Continuous Quality Improvement.
